Step 1 — Breast Analysis and Personalised Implant Selection

Before surgery, the surgeon performs a detailed analysis of the patient’s breast anatomy, including chest measurements, existing breast tissue quality and volume, inframammary fold position, and symmetry. Based on this assessment, the most suitable implant type, size, profile, and shape are selected to achieve a harmonious result tailored to each patient’s body.

Many clinics in Istanbul use implant sizers or 3D digital simulation systems, allowing patients to visualise their expected results before surgery.

Step 2 — General Anaesthesia

The procedure is performed under general anaesthesia. The patient feels no pain or discomfort and is continuously monitored by the anaesthesia team throughout the operation.

Step 3 — Incision and Surgical Access

The surgeon makes a small incision in one of three possible locations, depending on the case and patient preference:

• Inframammary fold (under the breast crease) — the most common approach, offering high precision

• Periareolar (around the lower edge of the areola)

• Transaxillary (through the armpit)

Each technique has its advantages, and the surgeon selects the most appropriate option for each patient.

Step 4 — Pocket Creation and Implant Placement

Through the incision, the surgeon creates a pocket where the implant will be placed. This pocket can be positioned in different anatomical planes:

• Subglandular (above the muscle)

• Dual plane / subfascial (partially under the muscle)

• Submuscular (fully under the muscle)

The choice depends on the patient’s tissue, implant type, and desired aesthetic outcome. Once the pocket is prepared, the implant is inserted, carefully positioned, and symmetry is checked.

Step 5 — Symmetry and Final Adjustment

After placement, the surgeon carefully evaluates symmetry, implant position, and overall aesthetic outcome. In some cases, the patient may be briefly positioned upright during surgery to help assess natural breast shape and balance.

Step 6 — Closure and Dressing

The incisions are closed using fine absorbable sutures to minimise visible scarring. A special post-surgical support bra is applied to stabilise the implants, reduce swelling, and help the implants settle into their final position.

Types of Breast Implants Available in Istanbul

Cohesive Silicone Gel Implants

The most commonly used implants worldwide. Filled with cohesive silicone gel that closely mimics natural breast tissue. They are soft, natural in appearance, and available in different levels of cohesivity. In case of rupture, the gel remains contained within the implant shell. Available in round and anatomical shapes.

Round Implants

Provide more fullness in the upper breast and a more pronounced cleavage effect. They are versatile, predictable, and widely used. They do not rotate inside the pocket, making them a stable option for many patients.

Anatomical (Teardrop) Implants

Designed to replicate the natural breast shape, with more volume in the lower part and a softer upper contour. They are ideal for patients seeking a subtle and highly natural result. They require a textured surface to prevent rotation.

Saline Implants

Filled with sterile saline solution. They allow minor volume adjustments during surgery and require smaller incisions. They are less commonly used due to a less natural feel compared to silicone implants.

Smooth vs Textured Implants

Smooth implants move freely within the pocket and are commonly used with round implants. Textured implants adhere slightly to surrounding tissue, reducing rotation risk and are typically used with anatomical implants.

Recovery Timeline

Days 3–7:
Discharge and transfer to the hotel. Gentle walking is recommended. Arm movements above shoulder level should be avoided.

Week 2:
Most patients can return home and resume light daily activities. Swelling and tightness gradually decrease.

Weeks 3–6:
Moderate activities can be resumed. Heavy lifting, intense exercise, and impact sports must be avoided. Implants begin to settle into their final position (“drop and fluff”).

From Month 2:
Gradual return to gym activities with medical approval.

3–6 months:
Final results become fully visible as swelling disappears and implants settle completely. Scars continue to fade over time.
